    New E-Worksheet: Caregiver Information

    A new, commissioned worksheet is available in the PassPorter's Club ... the Caregiver Information worksheet.

    Leaving the kids behind while you travel? Record the important information about your trip -- and your child's routines and activities -- for the caregiver. This worksheet lets you fill in the important travel dates and places at the top of the sheet. Below that are several fields for important phone numbers that the caregiver may need. Next is a chart to record routines for each of your children -- you can use our drop-down lists of typical routines or type in your own. The second page is dedicated to a grid where you can list your child's scheduled activities (i.e., soccer practice, Girl Scouts) and the dates. There's also a notes field at the bottom of both pages. You can use this worksheet for pets, too.
