Medical Information Form Hello! I had a question about the cruise documents. I have heard that you don't receive them until about 2 weeks before your cruise. I looked at the medical form online, and noticed that you need a doctor's signature. Does EVERYONE need this signature? I'm a student who has moved away to university and I don't live in the same city as my doctor, so this will take some planning! Thanks!

Re: Medical Information Form Hi! Yes, I live in Ontario, Canada. I looked at the form on the American Disney Cruise site, and it had a spot for a doctor's signature. I guess I should be safe and get one?

Re: Medical Information Form I thought the medical form was only if you have a condition that may affect you on the cruise, or if you have special needs of some kind. I had it filled out for my DDs and our doc signed it, but I wouldn't see any other reason for it.

Re: Medical Information Form I just confirmed with DCL. The medical document only needs to be completed and okayed by your physician if you have a condition that you want the cruise line to be aware of.
