Make sure to use the phrase "medically necessary" -- I've never had to have written proof, but I do always have the package with the prescription that states it must be kept cold. I'd guess that would be good enough if I'm asked.
FYI, the fridge will have to be brought up to the room after you're in (generally, they aren't in the room already) and it will need to be plugged in and cooled down before it will maintain a cool temp for your meds. You can either ask that the front desk or luggage storage keep the meds in their fridge, or you can fill the ice bucket in the room with ice and keep it in there until the fridge is cold enough.
We have it put on our requests on our reservation. When you get there, remind them at check-in. Then when you get to your room, you will call Housekeeping and they will deliver it.
We stay at AS Music every year. We have never been asked for proof. We keep two tiny bottles of insulin and some special liquid food for my very diabetic husband (and of course, a few Diet Cokes!) in the fridge.
But if you would feel better with a letter or note, ask your pharmacist or doctor to indicate that your meds require a fridge. You probably won't be asked about it, though.
