The Fast Five is determined by the first five users who click on the photo's icon. The names show up on the photo's database and the top 5 get the bonus entry. Yes:

Additionally, if you comment, then the photo appears on the photo home page and makes it quite easy for everyone to simply click on it without looking for it.

Does the Fast 5 bonus only apply to the daily Mystery photo and not any Bonus (Podcast / newsletter) or Facebook photos?

That's what I thought (and what I deduced from your description of the 'rules'). It's just that I thought someone had mentioned that they were one of the first 5 in getting one of the Bonus photos and wondered if the scope had been broadened! Thanks for clearing it up for me!
